BARBERSHOP TALKS WITH VIT4MOB

This Release Form is entered into by and between the undersigned participant ("Participant"), VIT4MOB Global, LLC, producer of the From Hurt 2 Healed Podcast, and DayeTime Photography.

I voluntarily agree to participate in the recording of the From Hurt 2 Healed: Barbershop Talk Podcast on June 26, 2026.

I understand that my participation may include:

• Audio recording

• Video recording

• Photography

• Live streaming (if applicable)

• Social media content creation

• Promotional materials and advertising

Grant of Rights

I grant VIT4MOB Global, LLC, the From Hurt 2 Healed Podcast, and DayeTime Photography the unrestricted right to:

• Record my voice, image, likeness, and statements.

• Photograph and videotape me during the event.

• Edit, reproduce, distribute, publish, display, and otherwise use such recordings and images.

These materials may be used in:

• Podcast episodes

• YouTube broadcasts

• Social media platforms

• Websites

• Marketing campaigns

• Educational materials

• Future promotional content

I understand that these materials may be used indefinitely without further approval.

No Compensation

I understand and agree that I will not receive compensation, royalties, or other payment for the use of my appearance, voice, likeness, photographs, recordings, comments, or participation.

Voluntary Participation

I understand that my participation is voluntary and that I may decline to answer any question during the recording.

Release of Liability

I release and hold harmless VIT4MOB Global, LLC, From Hurt 2 Healed Podcast, Victoria Bateman, DayeTime Photography, their affiliates, employees, volunteers, contractors, successors, and assigns from any claims, demands, damages, liabilities, or causes of action arising out of the use of recordings, photographs, video footage, or media content created during this event.

I have read this Release Form, understand its contents, and voluntarily agree to its terms.
